    Inline controls were a thing but i can’t see any here. I’d assume the circular part (both the skull and the buttons) are the CD unit, and it’s circular because the CD goes inside and takes up the full size. It’s as small as it can be while still fitting a CD.

    So the controls are on the unit itself, which is in his pocket.
