There’s what looks like cut marks down by the back of her knee, so could be to cover up self-harm scars as well.
Edit: zooming in looks kinda like just colourful lines, not scarring, sooooo 🤷
Would also be an uncommon place for sh. Not saying it’s impossible obviously, just not typical.

It looks like it’s not finished and there’s a figure below it. Normally the linework would be done first though.
I think that is just the line work - pretty sure the red is blood from the tatooist being bad at what they do/the person getting tatooed being a bleeder.

I’m guessing fire?
At first I thought it was Guts’ wolf armor in sketchy shadow style but the longer I look, the less sure I am.
I say it’s an angel fighting some kind of fire demon.
But there also seems to be a person hanging from the top of the fire (you can see a head, upper body, with the arms hanging down). So maybe the fire is hell, and the person being consumed by it, or rescued from it.
