Yeah, we were joking in our previous comment, we were being intentionally hyperbolic in order to be humorous as the joke’s expectation is that the ‘fantasy’ is going to be something else.

Yeah pretty much. Also there’s people doing HRT for reasons other than being transgender. There’s also people whose load sizes are different from what we’d expect based on their current hormones and gender expression.
I am of the opinion (or maybe it’s just that the evidence I have is) that cis people don’t tolerate cross-sex hormones, and I tend to think if you aren’t having a bad time on cross-sex hormones you’re some kind of trans. I get that the “still cis tho” is a bit like the “men having sex with men” label, but I also read between the lines with those folks. (If you think this is offensive, I think it’s at least important for clinicians to be able to read between the lines like this so HRT isn’t denied for people who probably do need it even if their self-conception is still resistant to being “trans”.)
